Antiviral drug, a synthetic analogue of thymidine nucleoside, which has a highly selective effect on herpes viruses.Inside virus-infected cells under the action of viral thymidine kinase, a number of consecutive reactions of acyclovir transformation in mono-, di- and triphosphate of acyclovir. Acyclovir triphosphate is inserted into the viral DNA chain and blocks its synthesis by competitive inhibition of the viral DNA polymerase.

Specificity and very high selectivity of action are also due to its predominant accumulation in cells affected by the herpes virus. Highly active against the virus Herpes simplex 1 and 2 types; a virus that causes varicella and shingles (Varicella zoster); of the Epstein-Barr virus (virus types are listed in order of increasing minimum inhibitory concentration of acyclovir). Moderately active against cytomegalovirus.

With herpes prevents the formation of new elements of the rash, reduces the probability of cutaneous dissemination and visceral complications, accelerates the formation of crusts, reduces pain in the acute phase of herpes zoster.

Pharmacokinetics:

When applying the ointment on intact skin: the absorption is minimal; not is determined in the blood and urine. Affected skin: moderate absorption; the patients with normal renal function serum concentration is up to 0.28 μg / ml, in patients with chronic renal insufficiency - up to 0.78 μg / ml. It is excreted by the kidneys (up to 9.4 % daily doses).

Indications:

Herpes simplex skin and mucous membranes, genital herpes (primary and recurrent); localized shingles (ancillary treatment).

Contraindications:Hypersensitivity.

Pregnancy and lactation:

Use during pregnancy is only possible if the intended benefit to the mother exceeds the potential risk to the fetus. If you need to take acyclovir during lactation, interruption of breastfeeding is required.

Dosing and Administration:

Ointment is applied to the affected skin 4-6 times a day (as soon as possible after the onset of infection). A strip of ointment 1.25 cm should be applied on 25 square cm of affected skin. It is important to begin treatment of recurrent infection during the prodromal phase or at the very onset of infection. Duration of treatment - at least 5 days, a maximum of 10 days.

Side effects:

With external application - soreness, burning, itching, skin rashes, vulvitis, allergic reactions,edema at the site of application.

Overdose:

At present, no cases of drug overdose have been reported.

Interaction:

An increase in the effect is noted with the simultaneous appointment of immunostimulants.

Special instructions:

Ointment should not be applied to the mucous membranes of the mouth and eyes.

Aciclovir ointment is effective for the prevention of herpes simplex recurrences. The effectiveness of treatment will be higher the earlier it was started (at the first signs of infection).

To realize the therapeutic effect of acyclovir, the state of the body's immune system is important. Patients with reduced immunity (with HIV / AIDS or after bone marrow transplantation) should receive systemic administration of the drug against the background of topical application of acyclovir ointment, as well as in case of severe and recurrent course of herpetic infection.

To prevent autoinoculation to other areas of the skin, use fingertips or rubber gloves.

Acyclovir does not prevent sexual transmission of herpes, therefore during treatment it is necessary to abstain from sexual intercourse, even with absence of clinical manifestations.

Acyclovir does not affect the ability to drive vehicles and perform work that requires an increased speed of psychomotor reactions.
