    Pharmacodynamics:

    It is a substrate for aerobic and anaerobic glycolysis, improves the antitoxic function of the liver, increases the contractile activity of the heart muscle, covers part of the energy expenditure, which, with a number of pathological conditions, is due to body reserves. The introduction of glucose solutions quickly replenishes the water deficiency. Entering the tissue, phosphorylated, turning into glucose-6-phosphate, which is actively included in many parts of the body's metabolism.

    Pharmacokinetics:

    It is absorbed completely by the body, the kidneys are not excreted (the appearance in the urine is a pathological sign).

    Indications:

    Hypokalemia, insufficiency of carbohydrate nutrition, toxicinfection, intoxication in liver diseases (hepatitis, cirrhosis, hepatic coma), hemorrhagic diathesis; dehydration (vomiting, diarrhea, postoperative period); collapse, shock.

    Isotonic glucose solution (5%) used to fill the volume of fluid in the cell and general dehydration, with extracellular hyperhydration.

    Hypertensive glucose solution (10%) Used for hypoglycemia and liver diseases; severe infectious diseases; various intoxications; hemorrhagic diathesis; for parenteral nutrition.

    Glucose solutions can be used both independently and for dilution of drugs.

    Contraindications:

    Hyperglycemia, hypersensitivity, hyperhydration, postoperative disorders of glucose utilization; edema of the brain, pulmonary edema, acute left ventricular failure, hyperosmolar coma.

    Carefully:

    Decompensated chronic heart failure, diabetes mellitus, chronic renal failure (oligo-, anuria), hyponatremia.

    Dosing and Administration:

    Isotonic glucose solution (5%) injected into the vein (drip) at a maximum rate of up to 7.5 ml (150 cap / min) (400 ml / h): 300-2000 ml of solution per day; hypertensive solution (10%) up to 60 cap / min (3 ml / min); the maximum daily dose for adults is 1000 ml.

    In adults with a normal metabolism, the daily dose of dextrose administered should not exceed 4-6 g / kg / day, i.e.about 250-450 g / day (with a decrease in the metabolic rate, the daily dose is reduced to 200-300 g), while the volume of the injected liquid is 30-40 ml / kg / day.

    Children for parenteral nutrition, along with fats and amino acids, 6 g of dextrose / kg / day are administered on the first day, followed by up to 15 g / kg / day. When calculating the dose of dextrose with the administration of 5% and 10% solutions, it is necessary to take into account the permissible volume of the injected fluid: for children with a body weight of 2-10 kg - 100-165 ml / kg / day, for children with a body weight of 10-40 kg - 45 -100 ml / kg / day.

    Rate of administration: at the normal state of metabolism, the maximum rate of administration to adults is 0.25-0.5 g / kg / h (with a decrease in the intensity of the metabolism, the rate of administration is reduced to 0.125-0.25 g / kg / h). In children, the rate of administration should not exceed 0.5 g / kg / h; which is 5% solution - about 10 ml / min - 200 drops / min (20 drops = 1 ml).

    Patients with diabetes mellitus are administered dextrose under the control of glucose in the blood and urine.

    Side effects:

    Hyperglycemia, fever, hypervolemia, acute left ventricular failure.

    At the injection site - the development of infection, thrombophlebitis.

    With an increase in the rate of administration - osmotic diuresis with loss of water, electrolytes and the development of hyperosmolar coma.

    Special instructions:

    For more complete assimilation of dextrose, administered in large doses, simultaneously with her, insulin is prescribed at the rate of 1 ME insulin for 4-5 grams of dextrose.

    When combined with other drugs, it is necessary to visually control the pharmaceutical compatibility.
